PanOptix Failure Over TIme?

Just wondering if anyone else has expeienced this. I'm 60 and in good health, had a PanOptix lens installed in my right eye in early 2021, and a cataract fixed. At first it was great, much clear vision at distance, middle, and near. Now in the past few months it seems to have totally failed - the only thing that's somewhat clear is distance - near and middle are completely blurry. I didn't have an annual eye exam last year due to moving, but am trying to get in to my optometrist who recommended it asap. I'm not even sure I'm legal or safe to be driving, and can only type this by getting close to the screen and using my left eye. I suppose it could be something unrelated. Thanks for any info.
